Look at FreshMeat and other places for CVS to SQL conversion utilities.
If my own project I would likely write up something quick in PERL, but I
assume there are hundreds out there already.

This isn't a problem with uploading (IE: the menu item under the larger
box), but a problem with the CVS import. You can use the "Or Location of
the textfile" for larger files. I do believe that PHP has a file size
limit which we can extend if there are problems.

The 'Insert data from a textfile into table' option has been removed
from phpMyAdmin on that server. Took me a while to find that option ;-)

For a beginning idea of the complexities of the way phpMyAdmin
implimented this feature, see:
http://www.mysql.com/doc/en/LOAD_DATA_LOCAL.html

The "where does the file get loaded, and what access permissions should
it have" are non-trivial issues to deal with. Had phpMyAdmin included the
very small CVS-to-SQL converter within it (Like the built in SQL-to-CVS
for data dumps), then this problem would not happen at all.
