I failed to duplicate this with version 3.23.36 
using your table definitions and your insert statement (appended below).
The database on Flora is 3.23.33
Also failed to duplicate it on Flora with a new smaller table.
None of the MySQL upgrade Change logs mention that bug.
Are you sure it is MySQL that is inserting that "set approve=0" clause?
What's in Submit2.php3?
